Overview

The Seattle Cancer Care Alliance (SCCA), located in Seattle, Washington, is part of a dynamic collaboration among three organizations known nationally and internationally for their patient care and research: Fred Hutchinson Cancer Research Center, University of Washington, and Seattle Children's. Over the past 20 years, these institutions have worked together to support their mission of adult and pediatric oncology patient care services, research and education.

 

The Patients Navigator for African American communities offers individualized assistance to patients, families and caregivers to help overcome health system barriers and facilitate timely access to qualified medical and psychosocial care. Patient navigators serve as a critical link to connect our diverse patient population to local and national resources to meet basic life needs while undergoing cancer treatment. These government and privately funded sources of support (medical transportation, housing, financial concerns, insurance coverage, medication co-pays), are crucial to enabling patients to engage in treatment from the point of diagnosis and throughout the cancer continuum. Furthermore, our patient navigators provide essential cultural brokering and emotional support that also enables our patients to receive the needed medical and holistic care they need. This particular position will focus upon African American patients/families but will cross cover other areas as needed.

Responsibilities

  • Assessing barriers to care and involve patients/families in creating potential solutions to problems, considering patient's physical and emotional status, reading level, health literacy, culture, language, and amount of information desired
  • Provide longitudinal support to patients throughout their treatment course
  • Identifying and arranging internal and/or external resources to meet patient needs. Continually expand knowledge of community and other resources, including immigration, DHSH, and financial resources
  • Serve as a link to clinical Social Work (and other supportive care services) to ensure patients are receiving the emotional and mental health care they need
  • Educating patients and caregivers on the multi-disciplinary nature of cancer treatment, the roles of team members, and what to expect from the health care system
  • Empowering patients by promoting self-management skills and providing information about resources and referrals
  • Encouraging active communication between patients/families and health care providers to help optimize patient outcomes
  • Assessing patients' capacity to self-advocate. Help patients optimize time with their treatment team by prioritizing questions, clarifying information, etc.
  • Contributing to patient navigation program development, implementation, and evaluation; follow through on special projects and assigned tasks as requested by supervisor
  • Demonstrating empathy and provide information and support within scope of practice; refer to licensed clinicians as appropriate
  • Adhering to organizational policies and procedures to ensure compliance with HIPAA, privacy, safety, and other regulations; document care provided in accordance with SCCA policies and procedures

 

In addition to the role/responsibilities detailed in the SCCA Patient Navigator II job description, the African American Patients Navigator will assume the following unique responsibilities as their orientation, capacity and patient demand allow. With guidance and support from their supervisor, the African American Patients Navigator will:

  • Reach out to current Black/African American patients in coordination with their existing Supportive Care service providers, and to incoming new Black/African American patients
  • Follow the Black/African American patients for the duration of their treatment, maintaining close relationships with patients and families, assessing and addressing emergent needs, and checking in even when there is not an identified episodic need
  • Serve non-African American patients as training needs require and capacity allows, until Black/African American patients-specific responsibilities fill their template. As needed and per the Supervisor, the incumbent will provide cross-coverage for other Navigators. As possible, other Navigation team members will provide backup to the incumbent.
  • Serve as an ambassador, educator and cultural broker to the health care teams and other SCCA staff so that Black/African American patients receive culturally appropriate care and services, and providers and staff develop improved cultural competence
  • Work closely with the Fred Hutch Office of Community Outreach and Engagement (OCOE) African American Patients Community Health Educator
  • Guide / participate in Black/African American patients outreach activities pertaining to cancer prevention, screening and treatment
  • Provide consultation and education to Black/African American patients, families and caregivers, SCCA providers and staff, and community members to enhance access to care, research and positive health oucomes for Black/African American patients

Qualifications

Required:

  • High school diploma or equivalent
  • Ability to communicate effectively and courteously with patients/families from diverse cultural backgrounds; sensitivity to the often stressful situations patients/families are facing; effective interpersonal skills
  • Ability to work well with diverse members of multidisciplinary team
  • Good written and oral communication skills; excellent organizational skills
  • Ability to work independently and shift easily from one task to another as needed. Ability to prioritize tasks, problem-solve daily, and complete work within deadlines
  • Strong organizational skills
  • Team player: timely communication, follows through on commitments, demonstrates reliability
  • Oral and written fluency in English
  • Working knowledge of community resources for the greater Puget Sound Region and beyond
  • Technical skills in Microsoft Office and other programs (EHR, etc.), as the job requires
  • Relate and work effectively with families, health care providers, social service agencies, and the public
  • Familiarity with cultural, health care and social practices of Black/African Americans
  • Successful experience in provision of community outreach and social services coordination to Black/African Americans
  • Interest in / prior experience with program development, innovation, experimentation
  • Current Washington State driver's license


Preferred

  • BA/BS in Social Work or other health related field
  • Active Washington State nursing or social work license
  • Experience with and/or ties to underserved populations
  • Demonstrated understanding of oncology /medical terminology
  • Two years working in a hospital or other healthcare setting, especially working with oncology patients and their support persons
  • Experience in community clinic settings
  • Prior participation in operations research activities
  • Oral and written fluency in a language other than English
  • Experience in working with underserved populations
