Responsibilities

The Patient Care Coordinator is a member of a dynamic team focused on coordinating all aspects of patient care. The role provides continuity of care by ensuring a smooth transition between care settings and by developing a relationship and trust with patients to identify and assist with each patient’s individual needs that may arise related to the care they receive. Additionally, the role coordinates and schedules surgeries and surgical appointments and collaborates with medical residents and surgeons

 

  • Coordinates and schedules surgical appointments and surgeries. Enters patients/surgery into scheduling program
  • Coordinates the schedules of the physicians, which includes clinic, and personal calendars, to ensure that the appropriate number and types of patients (consult vs. preoperative) are being seen
  • Works directly with identified new patients or directed established patients to obtain accurate demographic and financial information from self-pay and high financial risk patients.
  • Utilize real-time automated tools for insurance verification, eligibility, and benefits information; may require contacting insurance carrier.
  • Communicate with insurance carriers, study team, and/or other providers to obtain required information on the patient’s behalf.
  • Communicate, as needed, with patients and/or their families to obtain financial information and/or address account concerns.
  • Perform various account updates to patients’ accounts, as needed.
  • Accurately and courteously explain policies regarding financial assistance programs and assist patients/families in the completion of applications.
  • Pursue coverage opportunities for uninsured and underinsured patients; assist patients and families in the completion of financial assistance applications.
  • Responsible for daily identification of problem accounts and handles resolution.
  • Coordinate on-site support on a daily basis, including, providing financial counseling support, managing patient pre-authorizations, and preparing price estimates upon request from patient.

 

Qualifications

  • Ability to work independently, organize, and prioritize concurrent responsibilities and demand
  • Ability to communicate verbally in person, on the phone, and in writing in a clear and concise manner
  • Proficient in the use of word processing (MS Word), email, and data entry in a Windows-based environment
  • Demonstrated customer service skills
  • Ability to solve complex problems and meet patient needs
  • Ability to work in a high pressure, deadline oriented, and complex health care environment
  • Experience with Epic or other scheduling and registration software
  • Associates degree or equivalent experience required, Bachelor’s degree preferred.
  • Prior hospital scheduling/billing experience.
  • Minimum of two years of financial experience in a hospital or ambulatory setting.
